Prenatal Visits
Well Child Visits
Prenatal We encourage parents expecting their first child to visit during the third trimester to meet one of our pediatric professionals (physician, nurse practitioner, physician assistant), learn about our care practices, and tour our office.
Birth to 12 months Scheduled visits occur within a few days of nursery discharge, at 1-2 weeks, and at 1, 2, 4, 6, 9, and 12 months to assess growth and development, to provide routine immunizations, to screen for problems, and to answer parents' questions.
1 to 3 years Scheduled visits occur at 15 and 18 months, then at 2, 2.5, and 3 years, to complete the primary immunization 1-3 years series, to assess growth and development, to screen for problems, and to answer parents' questions.
4 to 5 years Scheduled visits occur at 4 and 5 years to assess growth, development, and readiness for school, as well as to complete the immunizations required for kindergarten entry..
6 to 11 years Scheduled visits occur annually to assess growth, school progress, participation in sports, dance, and other activities and anticipate the onset of adolescence.
12 to 14 years Scheduled visits occur annually to assess growth, health status, the changes associated with adolescent maturation, academic progress through middle school, participation in sports, dance, and other activities; and to provide required immunizations. Time is permitted for a confidential private visit of the teen with the pediatric professional.
15 to 18 years Scheduled visits occur annually through high school to monitor completion of growth and maturation, to assess health approaching college, work, or a military career, and to provide required booster immunizations. A confidential private visit of the teen with the pediatric professional is encouraged.
19 to 21 years One additional visit is recommended prior to the 21st birthday to reassess health status and to assist the youth, especially those with chronic medical conditions, with transition to adulthood and adult health care.
Special Visits
Sick visits Visits scheduled the same day they occur for any type of acute illness or other concern, typically a 15 minute visit.
Injury visit Visits scheduled emergently to deal with minor, non-life threatening injuries; requests for injury visits are triaged by a nurse to assure the child is not better seen in an Emergency Room.
Problem visits Visits scheduled in advance of the day they occur for any type of non-acute problem or concern; visits may be scheduled for 15 - 60 minutes depending on the nature of the problem.
Medication checks Visits scheduled for periodic review of long term medication use, primarily medications used to treat ADHD, Medication checks depression, anxiety, oral contraception, and other chronic medical conditions.
Sports exams Sports exams may be completed at a well child visit or at a separate visit (which is not covered by most insurance plans); sports forms may be completed based on a well visit done within a year prior to the date the form is signed.
Driver's exams Driver's exams may be completed at a well visit or at a separate visit (which is not covered by most insurance plans); the driver's exam may not be completed more than 6 months before the 16th birthday, but is valid for a year once it is performed.
Ear lobe piercing Piercing of the fleshy ear lobe with self-piercing earrings can be done at a well visit or at a separate scheduled visit. Ear piercing is not covered by insurance and must be paid for separately at the time of the visit.
Consultations
Lactation consulting Consultation with our lactation counselor for breast feeding mothers may be scheduled at the baby's first visit after nursery discharge, at the 2 week visit, at the 4 month visit, and at other times as determined by the mother and the lactation counselor. The lactation counselor is also available by telephone during regular
office hours.
ADHD/Mental Health Consultation appointments are available concerning attention deficit hyperactivity
disorder, depression, anxiety, and related developmental concerns. Typically information is sought in advance of these consultations, including school records, outside medical records, and questionnaires completed by parents, teachers, and
older children themselves
Developmental screening The Denver II Developmental Screening Test (DDST), a standardized developmental screening test, is carried Developmental screening out at well visits from 2 weeks to
5 years, to identify children with delays in their early development.
Autism Screening Children are assessed for symptoms of autism with the M-CHAT (Modified Checklist
for Autism in Toddlers Revised), a standardized questionnaire completed by the parent
at the 18 month well visit, and by informalassessment at all early well child visits.
Vision & Hearing Vision and hearing screenings are carried out at the 5 year well visit, and may be
Screening carried out at later well child visits for home-schooled children and other children
who do not receive the screening in the school setting.
Depression Screening A depression screening questionnaire is completed by adolescents at well visits from
12 years through 21 years of age.Children are assessed for symptoms of autism with
the M-CHAT (Modified Checklist for Autism in Toddlers Revised), a standardized
questionnaire completed by the parent at the 18 month well visit, and by informal
assessment at all early well child visits.
Nurse Visits
Allergy Injections Allergy injections may be given by one of our nurses based on orders provided by the
Allergist ordering the serum, following the schedule developed by the Allergist.
Flu Shots Influenza immunization shots are available by appointment at special "flu clinics" held
one Saturday each in September, October, November, and December (optional), as well
as at routine well child visits and scheduled nurse visits from September through
March each year.
Miscellaneous
Completion of Forms School health forms, day care forms, sports forms, camp forms, driver's license
forms, and other forms may be completed at no charge at well visits or special visits, or for an extra charge at any time. Please complete your portion of the form and bring the entire form with you, as we scan the completed and signed form into your child's electronic health record. Please allow at feast 2 days for the form to be completed, if not completed at a visit. The driver's license form must by signed in the office by the teen at the time the form is picked up.
After Hours Care Our practice is covered by an answering service from 5 pm to 9 pm Tuesday and
Thursday, with calls going to the pediatric professional on call. From 9 pm Monday
through Thursday until 8 am the next morning, and from 5 pm Friday until 8 am Monday, calls are handled by our Nurse Triage Service, with the pediatric professional
on call available for consultation during that period of time.
Patient Portal "My Child's Chart" is our Patient Portal that allows parents to access their children's
electronic health records, and teens 18 years of age and older to access their own
health record, from any electronic device at any time, and print out information, such
as immunization records, themselves. Our front desk staff will help you sign up to
access the Portal.
